All new clients, please complete these forms and bring them with you to your first session.   Please be aware that all information is kept and treated as highly confidential and all information is held in the strictest of confidence between Linda Wallace and her client. 

If you have scheduled a Distance Healing Session:

• Please return these forms to me at least 24 hours prior to your first session.  Forms not received at least 24 hours prior to your first session will result in automatic cancellation of your session.
• Payment for distance healing sessions is to be received to me at least 24 hours prior to your session.   Payment not received by me at least 24 hours prior to your session will result in automatic cancellation of your session.    I accept all major credit cards via Paypal as well as personal checks.   Please send Paypal generated payments to This e-mail address is being protected from spambots.
